Under the guidance of a Registered Nurse, this role plays a pivotal part in ensuring seamless patient care across both inpatient and outpatient settings. Whether on-site or through telephonic interactions, the primary objective is to maintain continuity of care and report findings promptly to the Care Management department Supervisor, Manager, or Director.

Responsibilities

  1. Deliver superior customer service by consistently demonstrating Optum’s commitment to quality, care, and concern for all stakeholders.
  2. Implement and adhere to current policies and procedures set by the Care Management department.
  3. Coordinate treatment plans and discharge expectations, engaging with attending physicians when necessary.
  4. Prioritize patient care needs, engaging in telephonic communication with patients, families, and caregivers as required.
  5. Identify and provide follow-up support for high-risk patients post-discharge from various care settings.
  6. Collaborate with physicians and team members to ensure effective implementation of care plans.
  7. Coordinate discharge provisions, including follow-up appointments, home health services, and transportation, to ensure continuity of care.
  8. Attend assigned Care Management Committee meetings and provide updates on patient status.
  9. Maintain effective communication with health plans, physicians, hospitals, extended care facilities, patients, and families.
  10. Provide accurate information to patients and families regarding health plan benefits and community resources.
  11. Ensure accurate and comprehensive documentation of patient interactions and care plans.
  12. Identify and refer patients to ambulatory care management programs to optimize clinical outcomes.
  13. Handle Optum patients’ protected health information (PHI) in accordance with HIPAA standards.

Required Qualifications:

  • Graduation from an accredited Licensed Vocational Nurse program.
  • Current LVN license in California.
  • Minimum of 1 year of recent clinical experience as an LVN/LPN.

Preferred Qualifications:

  • Minimum of 3 years of clinical experience as an LVN/LPN.
  • Minimum of 2 years of care management, utilization review, or discharge planning experience.
  • Experience in an HMO or Managed Care setting.

  • Hourly range: $19.47 to $38.08 per hour, based on education, work experience, and certifications.
