Responsibilities

- Identify and refer patients to ambulatory care management programs to optimize clinical outcomes.
- Handle Optum patients’ protected health information (PHI) in accordance with HIPAA standards.
Required Qualifications:
- Graduation from an accredited Licensed Vocational Nurse program.
- Current LVN license in California.
- Minimum of 1 year of recent clinical experience as an LVN/LPN.
Preferred Qualifications:
